Please fill out the form below if you have been disciplined, or know of a contractual violation which has occurred within your workplace, and would like the Local to investigate the circumstances.  Once we conduct an investigation, we will determine whether or not the circumstances warrant filing a grievance.  Please keep in mind there are contractual timelines in place for filing grievances with our various bargaining units, so inform us as early as possible following the discipline or contractual violation.  Although not required, please look at your Collective Bargaining Agreement before filling out the Statement of Grievant, and attempt to determine which part of the agreement has been violated.
